You will need to tell us:
- the address where the dog lives
- your contact details - only if you want to hear back from us.

Other helpful information
It is also helpful, but not essential, to tell us:
- the dog's name, colour and breed
- the dog owner's name
- a description of the dog's collar
- if the dog is aggressive
- if other dogs are at the property.
